Office 365: Microsoft Forms

Introduction

Microsoft Forms is a powerful tool, which is part of the Microsoft Office 365 subscription available to eligible faculty, staff, and students. Forms allows you to create surveys, quizzes, polls, and data collection forms with ease. It offers a range of options and features to customize and share forms, collect responses, and analyze data.

Here are some of its key benefits and features:

  • User-Friendly Interface: Microsoft Forms offers a simple and intuitive interface, making it easy to create forms without any coding or design skills.
  • Form Creation: Easily create custom forms with a variety of question types, including choice, text, images, rating, and date.
  • Templates: Microsoft Forms provides templates for quick form creation for common use cases like event feedback, registrations, and assessments.
  • Branching Logic: This feature allows form creators to direct respondents to different questions based on their previous answers, enabling a personalized survey experience.
  • Sharing: Forms can be shared via a link, QR code, email, or embedded in a webpage, making it accessible to a wide audience.
  • Real-time Collaboration: Multiple users can collaborate on form creation and analysis in real-time.
  • Integration: Forms integrates seamlessly with other Microsoft 365 applications like Excel, SharePoint, and OneDrive, facilitating data storage, sharing, and collaboration.

IMPORTANT!  Sensitive information including personally-identifiable information (PII) should not be stored in any online (cloud) file hosting platform, including Microsoft Forms.

Instructions

Signing on to Microsoft Forms

  1. Sign in to https://forms.office.com
  2. Click Sign in
    Sign In
  3. Type your Pace email address and click Next
    Pace Email Address
  4. Type your Pace password and click Sign in
    MyPace Password
  5. Enter in the 6-digit passcode generated from the Duo Mobile app or hardware token
    • Note: All faculty, staff, contractors, and students are required to use Duo MFA 2-factor authentication.

  6. Click Verify
    Duo Log In
    • Note: If prompted on whether to Continue and to remain signed in, click Yes.
       
  7. Once logged in successfully, the Forms Home page will display, along with buttons to create a new quiz, form, and other import or template options.
    • Note: The first time logging in, you will see a simple welcome screen with template categories to choose from.
