Financial Aid Forms and Applications
The following Financial Aid forms can be found linked on the Forms and Applications page of the Financial Aid website:
Loan Change Request Form
- This form is used to accept, decline or reduce a student's or parent's loan(s) for a particular loan period.
Package Adjustment Request Form
- This form is used to make a change to a student's housing status, home campus, to request a Fall only or Spring only financial aid package, to notify the department after returning to Pace University from a Leave of Absence or to notify the department that you will no longer be attending Pace University.
Financial Aid Appeal Form for Additional Aid Consideration
- Used to submit an appeal for additional aid consideration.
Satisfactory Academic Progress (SAP) Appeal Form
Parent PLUS Loan Application
- Federal loan application for Parents (biological, adoptive, or step-parents) of Dependent Undergraduate students to assist in paying Pace charges.
Graduate PLUS Loan Application
- Federal loan application for Graduate level students to assist in paying Pace charges.
Permission to Release Financial Aid Information Form
- This form is used to allow the Pace University Financial Aid Office to share financial aid information with third-party scholarship or study abroad providers.
Document Submission Form
- Documentation that cannot be submitted using one of our paperless forms should be submitted securely using the Document Submission Form.
