with your academic advisor’s approval. This form will allow you to...
To add/drop - a major or concentration
To add/drop - a double major
To add/drop – a dual degree
To add/drop – a certificate
A change of major form must be submitted within the first 2 weeks of the current semester
For forms submitted after the first two weeks, the change will be processed for the following semester
