Registration Action Form (PDF)

Summary

Information regarding registration action forms and processing.

Body

Registration action forms are used for when a student would like to add a course that is closed to their registration. The student will need to obtain approval from the professor, academic advisor, associate dean, or chair of the department that oversees the course in order to be added. However, if the room that the class is taught in is at maximum capacity, the student may not enter the course due to fire code regulations.

Registration action forms are used for when a student would like to withdraw from a course as well. This form will be processed if the student has received approval from professor, academic advisor, associate dean, or chair of the department that oversees the course. However, if the form was submitted after the withdrawal deadlines, the form will not be processed.
